Study on the Application Effect of Outpatient Follow-up Nursing Intervention in Elderly Diabetic Patients and Its Influence on Self-management Level

Purpose: To explore the application effectiveness of outpatient follow-up nursing interventions in elderly diabetic patients and their influence on self-management levels. Methods: 94 elderly diabetic patients from May 2022 to March 2023 were selected and divided into two groups of 47 each using envelope method. The control group received routine care, while the observation group received combined outpatient follow-up nursing interventions. Both groups underwent a 4-week intervention. Glycemic levels, self-management levels, awareness rates, and compliance were compared between the two groups. Results: After a 4-week intervention, the observation group showed lower FPG, 2hPG, and HbA1c levels compared to the control group (P<0.05); the observation group exhibited higher self-management level scores than the control group (P<0.05) after the 4-week intervention; the observation group also demonstrated higher awareness rates and compliance compared to the control group (P<0.05) after 4 weeks of intervention. Conclusion: Outpatient follow-up nursing interventions applied to elderly diabetic patients can lower patients' blood sugar levels, enhance self-management levels, and lead to higher awareness rates and compliance after discharge, suggesting its value for wider application and promotion.
