APPLICATION EFFECT OF THE REMOTE MANAGEMENT BASED ON INTERNET HOSPITAL AND FAMILY-SICKED BED MANAGEMENT BASED ON FOUR-PARTY LINKAGE MODE IN MAINTENANCE PERITONEAL DIALYSIS PATIENTS

Abstract

Objective: To explore the application effect of the remote management based on Internet Hospital and Family-Sicked Bed strategy based on the Four-Party Linkage in maintenance peritoneal dialysis (PD) patients. Methods: Baoan People’s Hospital (Group) innovatively using Internet Hospital as a platform to realize the remote management (RM) of PD patients, and launched the "resident-community-community health service center-hospital (group)" four-party joint disease management model. The nephrology specialist creates Family-Sicked Bed through this model. 150 medical insurance patients were divided into 3 groups including Non RM-PD group,RM-PD group, Family-Sicked Bed group using the random number table method.After half a year of intervention, comparing clinical indicators in the three groups, analyzed the time cost, economic expenditure and discussed whether the patient, medical staff and society could benefit in the three groups separately. Results:  The clinical outcomes including the rate of hospitalization, blood pressure, patients requiring antihypertensive medicines, number of antihypertensive medicines, capacity overload, edema, kaliopenia, peritonitis infection, Kt/V meet the standard, RM-PD group and Family-Sicked Bed group were superior to the Non RM-PD group(P<0.05).Multidimensional improvement including the quality of patients' home peritoneal dialysis,efficiency of medical staffs,social benefit,reducing the economic burden of patients were all achieved in RM-PD group and Family-Sicked Bed group. Non RM-PD group had the highest total annual medical cost after reimbursement, Family-Sicked Bed group had the lowest (mean ± SD)(Non RM-PD=39,383±2678, RM-PD=34,369±2898, Family-Sicked Bed=25,250±2763, based on Chinese Yuan CNY, P<0.05) .Conclusion: Remote management based on internet hospital and Family-Sicked Bed management based on Four-Party Linkage in peritoneal dialysis patient are effective and sustainable.
