ASSESSING THE CLINICAL EFFICACY OF FISTULA STRIPPING DRAINAGE FOR HIGH SIMPLE ANAL FISTULA IN ATHLETES

Abstract

Objective: This study aims to evaluate the clinical efficacy of fistula stripping and drainage with internal port ligation in the treatment of high simple anal fistula, particularly in athletes, who may require faster recovery times and return to physical activity. Methods: A total of 70 cases of high simple anal fistula treated in 2022 were analyzed. These cases were divided into two groups: 35 cases received traditional anal fistula cut and hang treatment (control group), and 35 cases underwent fistula stripping and drainage with internal port ligation (treatment group). We assessed recovery time, inflammatory marker levels, clinical efficacy, complication rates, pain scores (NRS), Wexner scores for continence, and anal function index scores. Results: The treatment group exhibited significantly shorter recovery times for first ambulation, hospitalization, and wound healing compared to the control group. After 3 months, the treatment group showed lower levels of inflammatory markers (PCT, IL-6, TNF-alpha, CRP) than the control group. Additionally, the treatment group had a higher excellent rate of clinical efficacy, lower incidence of complications, lower NRS scores at various time points post-treatment, and a lower Wexner score. Anal function index scores, including anal resting pressure, anal maximum systolic pressure, rectal sensory capacity, rectal maximum capacity, and rectal compliance, were significantly higher in the treatment group. Conclusion: Fistula stripping and drainage with internal port ligation demonstrated superior clinical efficacy in treating high simple anal fistula, especially significant for athletes, offering better outcomes, reduced inflammatory response, faster recovery, and improved anal function. This method allows athletes to return to their physical activities sooner while ensuring lower complication rates and enhanced quality of life post-treatment.
